This patch is for BZs: 253989, 35606, and 245823. It is a port of what
will be in 2.6.24 and one patch that was sent for 2.6.25 when the
feature window opens.

It fixes the following bugs:

During heavy writes we would see connection drops because a r2t was not
handled (the upstream patch is a little heavy and we could have just
moved the locking around the r2t send path, but the patch was well
tested from the vendor and does not cause a performance regression and
is upstream).

Bounce buffers are not needed, because the driver does kmap when it
needs to touch a buffer and it has no hardware limits as it is a
software driver.

Userspace current handles all nop traffic, but during boot or shutdown
when iscsid is not up if we do not respond the target will drop the
session on us. Also putting it in the kernel allows us to check the
transport, like how scsi_transport_fc does, before the scsi eh is run,
and we avoid running that for transport problems.
This one is not upstream yet. It is in the iscsi maintainer's tree
(which is us) and was sent to James for 2.6.25 when the feature window
opens.

The patches were tested by me with the following targets: netapp,
equalogic, iqstor, opensolaris (running in vm), iet and istgt/tgt, on
x86_64 boxes and 32 bit boxes with and without highmem, by running the
open-iscsi/test/regression.sh test (this runs bonnie++ and disktest and
mkfs with different iscsi settings).

I have also run the check-kabi patch over this to check for kabi
regressions.
