BZ 253267 Update qla2xxx - PCI EE error handling support

Additional support for handling and recovering from PCI errors.

Applies cleanly to 2.6.18-53.el5. Tested at the factory.

Not all of the upstream patch is applicable to a 2.6.18 kernel
so this is a partial backport of commit: 14e660e677ddd3574247495aae4ef63eb8899072
